by booking an appointment and receiving services at haus of rocky, you acknowledge and agree to the following:
 

medical disclosure
clients are responsible for providing complete and accurate medical history, including medications, allergies, medical conditions, and pregnancy status. failure to disclose relevant information may increase the risk of adverse outcomes and releases haus of rocky and its providers from liability related to undisclosed information.
 

results disclaimer
all aesthetic treatments carry inherent risks and individual results may vary. haus of rocky does not guarantee specific outcomes, as results depend on individual factors and adherence to pre- and post-care instructions.
 

informed consent
by proceeding with treatment, you confirm that you have reviewed and signed all required consent forms, have had the opportunity to ask questions, and understand the nature, risks, benefits, and alternatives of the procedures performed.
 

post-treatment care
clients agree to follow all pre- and post-treatment instructions provided by their provider. haus of rocky is not responsible for complications resulting from failure to follow recommended aftercare.
 

liability limitation
haus of rocky and its providers are not liable for known risks, adverse reactions, or outcomes associated with aesthetic procedures when appropriate protocols are followed.
 

no refunds
all services rendered and products purchased are final sale and non-refundable.
 

appointment policies
appointments are reserved specifically for you. late arrivals, cancellations, or no-shows may be subject to fees in accordance with clinic policy.
 

right to refuse service
haus of rocky reserves the right to refuse or discontinue treatment at any time if it is deemed unsafe or not in the client’s best interest.
 

privacy
all client information is kept confidential and handled in accordance with applicable privacy laws, including HIPAA.
